New Patient Registration

Please complete the On-Line Registration form and the New Patient Questionnaire.

Registrations cannot be processed without a completed New Patient Questionnaire.

New Patient Questionnaire

Completed forms should be emailed to: fife.f21204leuchars@nhs.scot and will be processed with 5 working days. If there is any reason we cannot process your application we will let you know via email.

You will be asked to provide valid identification. This can be a driving license, Passport, birth certificate or any other means of proof of identity. 

Patients are not tied to any one doctor and can request to see any of our doctors. Just ask for the doctor you would like to see when you book an appointment.

All new patients are offered a nurse consultation so that we can meet each other, learn your medical history and give you a simple check-up. 

We strongly recommend you take advantage of this service.
