Registering with a GP practice
You can check if you live within our practice boundary using this map.
To register permanently, you’ll need to complete a registration form and a new patient questionnaire. You can either:
- Fill out the registration form and new patient questionnaire in person
- Download the registration form and new patient questionnaire, complete them, and then email them to the GP practice
When you register with us, it’s recommended that you provide information to confirm your:
- identity
- address
This means you can be matched to your existing NHS records. You shouldn’t be delayed or refused registration at a GP practice if you can’t provide identification or proof of address.
Anybody in Scotland can access primary care services at a GP practice without charge.
Please complete the Registration form and the New Patient Questionnaire. Registrations cannot be processed without a completed New Patient Questionnaire. Completed forms should be emailed to fife.f21204leuchars@nhs.scot and will be processed with 5 working days. If there is any reason we cannot process your application we will let you know via email.
You will be asked to provide valid identification. This can be a driving license, Passport, birth certificate or any other means of proof of identity.
Patients are not tied to any one doctor and can request to see any of our doctors. Just ask for the doctor you would like to see when you book an appointment.
All new patients are offered a nurse consultation so that we can meet each other, learn your medical history and give you a simple check-up. We strongly recommend you take advantage of this service.
Registering as a Temporary Resident
You can only register with a GP practice if you’re staying in the area for more than 3 months. If you’re not, you may still be treated but as a Temporary Resident.
Please complete a Registration Form and in the top right corner, select 'NO', you will not be in the area for more than 3 months. We will need the address you are staying at temporarily, your regular home address and your regular home surgery details.
Accessing someone else’s information
You can give permission for someone else to act on your behalf. This can include making/cancelling appointments, ordering prescriptions, accessing test results and obtaining your medical information,
To do this, you must give us permission in writing and we can add this to your records. You can write a letter or submit this via email at Fife.F21204Leuchars@nhs.scot
Accountable GP
From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.
The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.
- Patients do not need to see their named GP when they book an appointment with the practice.
- Patients are entitled to choose to see any GP or nurse in the practice.
