Unfortunately we have currently suspended requests from patients looking to register with us who are already registered with a local Practice. This is due to an unsustainable increase over recent months and we can now only accept registrations from patients who are new to the area and not already registered with a local practice.

New Patient Registration

To reduce the spread of Covid 19 by limiting the volume of traffic to GP practices and reducing the amount of paper handled, there has been a change to the Patient Registration process which allows patients to complete a registration form online and then email it to the practice.

Please Complete our New Patient Questionnaire

Completed forms should be emailed to: fife.f21204leuchars@nhs.scot

You should receive a confirmation e-mail letting you know if your application has been accepted or not, and if not the reason why.

You will also need to complete a Patient Questionnaire which can be downloaded using the link below.

You will be asked to provide valid identification. This can be a driving license, Passsport, birth certificate or any other means of proof of identity. 

Patients are not tied to any one doctor and can request to see any of our doctors. Just ask for the doctor you would like to see when you book an appointment.

All new patients are offered a nurse consultation so that we can meet each other, learn your medical history and give you a simple check-up. 

We strongly recommend you take advantage of this service.
